Severe precapillary pulmonary arterial hypertension (PAH) may present with preserved resting oxygenation and minimal routine imaging abnormalities. This case study illustrates how disproportionate exercise-induced hypoxaemia and isolated diffusing capacity reduction can reveal severe precapillary PAH.
